FormMail: 403 Forbidden on Submit

Having in issue while trying to work with formmail, when I try a test submission I'm coming up with this error:

Forbidden
You don't have permission to access /formmail.php on this server.


Here's my code for the test form:

<input type="hidden" name="env_report" value="REMOTE_HOST,HTTP_USER_AGENT">

Could anyone offer some insight as to what I'm doing wrong?

Remove this (unless you uploading files):

Enctype="multipart/form-data"


Capitalize the entire word POST.

These are not defined nor are they variables:

<input type="hidden" name="env_report" value="REMOTE_HOST,HTTP_USER_AGENT">


Whats this do?

<input type=hidden name="email">


Try using the FULL URL to the script also to make sure the script isnt hiccupping.
