trying to install CZUser InfoV5 Universal Block

while trying to install the user info block, i received this error:

FAILED to ALTER TABLE nuke_session ADD module varchar(30) NOT NULL default '';
FAILED to ALTER TABLE nuke_session ADD url varchar(255) NOT NULL default '';

do i need to edit the mainfile.php prior to trying to use the install script? or is this a specific mysql issue that i need to fix? i've been trying to find a working user info block since mine stopped working.

The tables were not added. Did you follow step #5 of the install file?

If you already have had it installed then those fields are already added just disregard that error and move on to the next step in the install.
